Do you have any children? (List ages) If no, are you planning to have children in the future?

Are you married or single? If married or involved in a long-term relationship, does your partner approve and feel enthusiastic about getting a dog?

Where do you live? (city, country, subdivision, home, apt, condo) Do you own or rent? If you rent, does the landlord allow large pets? What type of property do you live on? Is the area fenced? If so, what kind, height, etc. How will the dog get exercise?

Would you allow us to visit your home by appointment?

Please provide us with personal and veterinarian references including phone numbers.

Have you ever raised and trained a puppy before?

What will you do with the dog while you are at work, or away on vacation?

Have you ever owned an Alaskan Malamute? What happened to it?

What books or information have you read about Alaskan Malamutes?

Do you own other pets? Please list type, age, sex, and if spayed or neutered.

Can and are you willing to commit to a long-term relationship with this dog (12+ years)?

Are you interested in a male or female and why? Is this puppy for showing or pet/companion? Are you willing to spay/neuter your pet puppy?
